For families · Before the phone rings

The first call from the care home. What they'll ask you.

Admission day, a fall, a new prescription: when the home calls, they need answers quickly, and you may be at work, on the road or three states away. These are the questions families get asked most, so you can have the answers ready.

What the home needs

Eight questions to be ready for

"What is she taking, and when?"

Every medicine and supplement, with dose, timing and who prescribed it.

"Any allergies?"

Medicines, foods and materials, and what happens if there's a reaction.

"Who is his doctor?"

Primary doctor and specialists, with phone numbers, plus the preferred pharmacy.

"What insurance does she have?"

Plan names and member numbers for Medicare, supplements, Part D and any long-term care policy.

"Who can make decisions?"

Who holds the healthcare power of attorney, and how the home can see it.

"Are there wishes we should know?"

An advance directive, any do-not-resuscitate order, and what matters most to your parent.

"Who do we call, and in what order?"

Two or three family contacts, with the hours each can be reached.

"Anything we should watch for?"

Recent hospital stays, falls, memory changes, or anything that's been different lately.

Have the answers in one place, and share only what they need.
